Only new employees are currently eligible to enroll. Open enrollment for all full time employees begins in May.

Information and Enrollment Form for Life, STD, and LTD

Critical Illness and Accident Insurance – If you wish to add either of these coverages, please use the combined application form.

Combined Application Packet – includes application for all five coverage types (on one form) plus Health Statement form – [do action=”doclink” tree=”Benefits” name=”2014-2015EnrollmentAssurant.pdf” title=”Download Application Form”/], required for any new coverages.

Return application, and health statement if required, to Cindy Felisberto within 31 days of starting as a full time employee.  

Voluntary benefits are paid in advance.  Costs for coverage are the responsibility of the employee.  Withholdings for new coverages, elected during open enrollment, will begin July 15, 2015 or when coverage is approved.  Withholding amount may be adjusted during first month of withholdings in order to meet the advance payment requirements.

CURRENTLY PARTICIPATING EMPLOYEES

If you are already participating in any of the Assurant voluntary benefits you will receive a mailing showing current coverages and projected costs for the following year in early May.  To make changes to existing coverages, please submit the form that is included in that mailing.  If you want to add any coverage that you do not currently have, please use the enrollment packet posted here, for them.  Even if you are not changing your coverage, you may change your HealthySolutions Selection.

NON-PARTICIPATING EMPLOYEES

Please review the information and if you would like to begin participating, submit the enrollment information during the open enrollment period.

ABOUT VOLUNTARY BENEFITS

All voluntary benefits are paid for via payroll withholding and employees pay 100% of the cost.  Withholdings are made the month preceding the coverage month.

Benefits offered include life, short term disability, long term disability, critical illness and accident coverage.  Disability plans are income protection plans.
